Lighthouse Social Worker

Summary

The Lighthouse Social Worker is a trained and compassionate professional, preferably with psychiatric/mental health experience, who understands the benefit of working as a part of a collaborative integrated team to provide consistent, sustained mental health support including behavioural interventions to clients in their apartment/residence.

Working as part of a team, the Lighthouse Social Worker will provide support to fifteen individuals requiring a medium-high level of residential support; clients may be housed within The Lighthouse or in the broader Saskatoon community. The focus of the medium-high residential support beds is intensive visiting support that might be daily and increasing or decreasing in frequency as the client requires. The Social Worker will work in conjunction with the Lighthouse care team, and with SHA service providers such as community recovery teams and/or other mental health and addictions services.

Job Duties

Using a multi-disciplinary skill set, the Lighthouse Social Worker must be able to exercise the following skills:

  • Locate and access housing based upon service participant preferences as well as affordability and appropriateness, assisting the service participant in signing the lease, and providing direct assistance in helping the service participant move into the apartment with furnishings and basic needs in place;
  • Provide support to clients in the development of daily living skills i.e. laundry, meal preparation/purchasing, hygiene, financial budgeting, cooking groups and development of coping skills necessary to attain greater stability and independence, improved functional ability and higher quality of life
  • Participate and work collaboratively in the completion of a client recovery plan with SHA designate and client within seven days of admission according to SHA standard of care; focus of the care plan is to transition to the highest level of independence which requires attention to ongoing functional assessment of activities of daily living and matching goals set with clients to work towards independence
  • Work with clients wherever they are in their recovery journey, being responsive to the needs of each individual, encouraging engagement with existing supports including psychiatry, mental health and addiction clinical teams, community recovery teams, peer support, and service providers, as well as development of new supports
  • Utilize a strengths based orientation that acknowledges that clients have strengths and resiliencies to determine their own meaning and goals which leads to increased client motivation and independence
  • Ensure a transition plan is completed to ensure continuity of care when client transitions to their next community residential support environment
  • Participate in client case conferencing as requested by the client’s care team
  • Safely accompany service participants to appointments in community and regularly conduct home visits;
  • Assist client in accessing services including but not limited to: health, mental health and addiction services; income supports; socio-recreational activities; services to persons with development delays and/or cognitive functioning issues; cultural, spiritual and/or religious services;
  • Create person-centred case plans with tangible goals related to housing stability, timeliness, and explicit accountabilities;
  • Establish crisis plans and risk assessment plans with each individual being supported in housing, and cater housing supports relative to the information gleaned from these plans
  • Engage in motivational interviewing and active listening;
  • Use a range of engagement and change motivation techniques with persons that may miss appointments or withdraw during engagement;
  • Prepare complete documentation and case notes within 24 hours of engagement with each service participant;
  • Organize and promote a range of meaningful daily activities for service participants to engage within, especially as it relates to accessing mainstream services;
  • Interact effectively and professionally on tenancy matters with the landlords of the service participants if they are required to move, while maintaining services and supports during the interim period;

Requirements

  • Candidate must have a social work degree and be licensed with a regulatory body and be in good standing
  • Preference given to candidates that have psychiatric/mental health experience
  • Training in Non-Violent Crisis Intervention, First Aid/CPR, Suicide Intervention, Motivational Interviewing would be an asset
  • Staff must have effective communication, both verbally and in writing to ensure that staff can interact effectively with clients to meet their care and psychosocial needs in a respectful and collaborative manner
  • Staff must be able to model healthy and appropriate way of interacting with others, problem-solving, assertion and self-care.
  • Staff must be physically and mentally capable to perform assigned duties and services.
  • Candidate must have an understanding of mental illness, mental health symptomology, and be able to work with care/case management plans.
  • Must be able to develop strong working relationships with client population, be able to relate to individuals making significant life decisions, and be supportive of individuals’ increasing independence.
  • Must be able to prepare written reports, data tracking and maintain client documentation. Basic keyboarding and computer literacy required.
  • Must possess a valid driver’s license and have access to a reliable vehicle

  • May be exposed to infectious waste, diseases and conditions
  • Manual dexterity required to use computer and peripherals.
  • Physical activity required including walking, standing
  • Work at a fast pace with unscheduled interruptions
  • Mostly day-time, weekday hours; some evening or weekend hours may be required
